We provide two main categories of in-home care throughout New Hampshire:
Personal Care & Support Services (Non-Medical): Assistance with bathing, grooming, dressing, and toileting; household support including light housekeeping, laundry, meal preparation, and errands; companionship and socialization; mobility support and transfer assistance; and respite care for family caregivers.
Skilled Nursing Services: Comprehensive nursing assessments; medication management and administration; wound care and dressing changes; IV therapy and PICC/Midline care; catheter insertion and maintenance; tube feeding (enteral nutrition) management; and chronic disease monitoring and education for conditions like diabetes, heart failure, and COPD.

Our care team includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Licensed Nursing Assistants (LNAs), Home Health Aides (HHAs), and Personal Care Aides (PCAs). All caregivers are thoroughly screened, trained, and supervised. Personal care staff work under the direct supervision of our Registered Nurses, who conduct regular supervisory visits to ensure quality and safety.
Yes. All of our direct care staff receive specialized dementia care training in compliance with New Hampshire law (RSA 151:47-51). This includes training on communication strategies for cognitively impaired clients, management of behavioral challenges, safety precautions, wandering prevention, and fall prevention specific to dementia. Our caregivers complete initial dementia training and ongoing annual education to maintain their skills.
Yes. We provide respite care to give family caregivers temporary relief. This can include any combination of personal care, companionship, and safety supervision provided for a designated block of time so you can rest, attend to personal matters, or simply recharge. Respite care can be arranged for a few hours, a full day, or longer periods.

For clients who need skilled nursing, a physician’s order is required before services can begin. For personal care services, our RN assessment and care plan are completed before the first visit.
It depends on the type of service. For skilled nursing services (wound care, IV therapy, medication management, etc.), a written physician’s order is required before care can begin. For non-medical personal care and companionship services, a physician’s order is generally not required, though our Registered Nurse will still conduct an initial assessment and develop a care plan before services start.
For personal care services, care can often begin within a few days of your initial contact, once our RN assessment and care plan are complete. For skilled nursing services, the timeline depends on receiving the physician’s orders and completing the comprehensive nursing assessment, which must occur within 48 hours of referral or on the physician-ordered start date. We always prioritize urgent situations.
As a client of NH Care Center, you have important rights protected by New Hampshire law (RSA 151:21-b), including the right to be treated with respect and dignity; to be informed about your caregivers’ qualifications; to participate in developing your care plan; to refuse treatment; to be free from abuse, neglect, and exploitation; to have your personal information kept confidential; to review your clinical records; to be informed in writing about all charges before services begin; and to be free from restraints. You will receive a written copy of the full Home Care Clients’ Bill of Rights before care begins.
You may file a complaint with our agency at any time without fear of reprisal. Contact our Administrator at 143 North Main St, Suite 202 A, Concord, NH 03301. We will acknowledge your concern within 48 hours and provide a written resolution within 7 days.

Yes. You have the right to refuse any treatment or service. If you choose to refuse, we will inform you of any potential medical consequences of that decision. Your choice will be respected and documented, and refusing a service will not result in discharge from our agency or any form of retaliation.
You can only be discharged or transferred under specific circumstances defined by New Hampshire law, such as when your needs exceed what we can safely provide, when care plan goals have been met, or in cases involving safety concerns. You are entitled to at least 21 days written notice before any non-emergency discharge, including the reason, effective date, your right to dispute the decision, and contact information for the Long-Term Care Ombudsman. You have the legal right to dispute any agency-initiated discharge.
Costs vary based on the type and frequency of services needed. We believe in full financial transparency — as required by New Hampshire law, you will be informed in writing of all charges, including what insurance covers and what you are expected to pay, before services begin. We accept various payment methods including private pay (self-pay), long-term care insurance, and may work with certain Medicaid waiver programs. During your initial consultation, we can help you understand your options and assist with insurance verification. Our goal is to make quality home care accessible to every family that needs it.
Every caregiver undergoes thorough screening before employment, including a criminal record check and verification against the state registry to ensure no history of abuse, neglect, or exploitation. All caregivers must meet specific health requirements, hold required certifications for their role, and complete comprehensive orientation covering client rights, safety procedures, and care protocols. Our clinical staff hold current New Hampshire licenses.
Yes. All personal care staff work under the supervision of our Registered Nurses. For clients receiving skilled nursing and aide services, an RN conducts supervisory visits regularly to assess the aide’s performance and ensure the care plan is being followed. For clients receiving only personal care, RN supervisory visits occur on a scheduled basis. If any nursing tasks are delegated to support staff, the delegating RN provides specific training, evaluates competency, and provides ongoing oversight.
Your comfort and satisfaction are our priority. If you feel your caregiver is not the right fit, contact us and we will work to reassign a different caregiver. You will never face any negative consequences for requesting a change, and your care will continue uninterrupted during the transition.
No. In accordance with New Hampshire law (RSA 151:21), NH Care Center does not deny admission, care, or services based on race, color, national origin, religion, sex, gender identity, sexual orientation, disability, age, or source of payment.
We take your privacy seriously. Your clinical records and personal information are strictly confidential and protected in compliance with HIPAA regulations. Access to your records is limited to authorized clinical staff directly involved in your care. You have the right to request copies of your records at any time, and we will never share your information without proper authorization.
We understand that caring for a loved one affects the entire family. Our services include respite care so you can take a break, regular communication and updates about your loved one’s care, participation in care plan development and revisions, education on caregiving techniques, and coordination with your loved one’s physicians and other providers. We view families as essential partners in the care team.
Absolutely. New Hampshire law guarantees clients and their representatives the right to participate in developing and revising the Plan of Care. We encourage family involvement at every stage — from the initial assessment through ongoing care. You will be informed of any changes to the care plan before they are made, and your input is always welcome.
