Do not send PHI by email. Standard email is not encrypted end-to-end. Send only basic intake information (patient initials, payer source, anticipated discharge date, requesting party name and callback). Our RN will call you back to take protected health information over the phone or via a secure channel.

Hand off in person or by fax

For hospital discharge planners and SNF social workers preferring paper referrals: call us first and we'll coordinate the right secure handoff method for your facility — often a brief in-person discussion at discharge or a transmission through your existing secure channel.

Our intake process

    Call or basic email reaches clinical leadership directly. We confirm receipt within one business day (often within hours).

    RN assessment within 48 hours

    A licensed RN conducts an in-home assessment with the patient and family. Urgent same-day discharges considered case-by-case.

    Plan of care developed

    Personalized care plan based on assessment findings, referring clinician's orders, and family input. Written cost estimate provided per NH RSA 151:21-b before any services begin.

    Care begins

    Services typically start within days of the assessment — or within hours for urgent discharges. RN supervisory visits scheduled per care plan acuity.
